7a0efe7da5 xv: use embedded private instead of pointer
The private struct is pretty small and it needs to be allocated anyways,
so save an extra allocation by directly embedding it.

34c3a9c7e2 treewide: fix serverGeneration int type mismatch
The global (exported) serverGeneration field is `unsigned long`, while
many other places copy it and compare it two other integer types, eg.
plain `int` (which is signed). Even if it's unlikely ever reaching such
high number of generations that it will ever make trouble, it's still
a good idea to clean this up and use the same type everywhere.

For clearity, introducing a typedef `x_server_generation_t` which is
used everywhere, instead of raw `unsigned long`.

dda64fcadf xext: replace xallocarray() by calloc()
Only key difference that calloc(), in contrast to rellocarray(),
is zero-initializing. The overhead is hard to measure on today's
machines, and it's safer programming practise to always allocate
zero-initialized, so one can't forget to do it explicitly.

bee797a348 Xext: xv: untwist Xinerama handling
The current way of switching between Xinerama and single-screen handlers
is quite complicated and needs call vector tables that are changed on
the fly, which in turn makes dispatching more complicated.

Reworking this into a simple and straight code flow, where individual request
procs just look at a flag to decide whether to call the Xinerama or single
screen version.

This isn't just much easier to understand (and debug), but also removes the need
or the call vectors, thus allowing further simplification of the dispatcher.

c96901a85a Xext: use calloc() instead of malloc()
Using calloc() instead of malloc() as preventive measure, so there
never can be any hidden bugs or leaks due uninitialized memory.

The extra cost of using this compiler intrinsic should be practically
impossible to measure - in many cases a good compiler can even deduce
if certain areas really don't need to be zero'd (because they're written
to right after allocation) and create more efficient machine code.

The code pathes in question are pretty cold anyways, so it's probably
not worth even thinking about potential extra runtime costs.

ef62929f58 treewide: NULL-protect ScreenRec->DestroyPixmap() calls
Right now, we're assuming that even when deep nesting involved, the proc
vector is always set to a valid function. One the one hand it requires
extra dummy procs in some cases, OTOH it's making upcoming refactoring
of the code flow unnecessarily complex.

The big plot (of subsequent commits) is splitting out the extension's
(and possibly subsystem's) special logic out of the wrapping chain and
let them be executed independently from the DDX/drivers - when applicable
even only when the pixmap is really destroyed (not just unref'ed).
(At some later point, it might even become be actually a valid situation
that DestroyPixmap vector really being NULL.)

fb696a7d7b rename old symbol PANORAMIX to XINERAMA
PANORAMIX was the original working title of the extension, before it became
official standard. Just nobody cared about fixing the symbols to the official
naming.

For backwards compatibility with drivers, the old PANORAMIX symbol will
still be set.

a917f6a8a8 drop obsolete HAVE_DIX_CONFIG_H
The symbol controls whether to include dix-config.h, and it's always set,
thus we don't need it (and dozens of ifdef's) anymore.

This commit only removes them from our own source files, where we can
guarantee that dix-config.h is present - leaving the (potentially exported)
headers untouched.

b79f32b57c Xext: free the XvRTVideoNotify when turning off from the same client
This fixes a use-after-free bug:

When a client first calls XvdiSelectVideoNotify() on a drawable with a
TRUE onoff argument, a struct XvVideoNotifyRec is allocated. This struct
is added twice to the resources:
  - as the drawable's XvRTVideoNotifyList. This happens only once per
    drawable, subsequent calls append to this list.
  - as the client's XvRTVideoNotify. This happens for every client.

The struct keeps the ClientPtr around once it has been added for a
client. The idea, presumably, is that if the client disconnects we can remove
all structs from the drawable's list that match the client (by resetting
the ClientPtr to NULL), but if the drawable is destroyed we can remove
and free the whole list.

However, if the same client then calls XvdiSelectVideoNotify() on the
same drawable with a FALSE onoff argument, only the ClientPtr on the
existing struct was set to NULL. The struct itself remained in the
client's resources.

If the drawable is now destroyed, the resource system invokes
XvdiDestroyVideoNotifyList which frees the whole list for this drawable
- including our struct. This function however does not free the resource
for the client since our ClientPtr is NULL.

Later, when the client is destroyed and the resource system invokes
XvdiDestroyVideoNotify, we unconditionally set the ClientPtr to NULL. On
a struct that has been freed previously. This is generally frowned upon.

Fix this by calling FreeResource() on the second call instead of merely
setting the ClientPtr to NULL. This removes the struct from the client
resources (but not from the list), ensuring that it won't be accessed
again when the client quits.

Note that the assignment tpn->client = NULL; is superfluous since the
XvdiDestroyVideoNotify function will do this anyway. But it's left for
clarity and to match a similar invocation in XvdiSelectPortNotify.

dc7ceda90f dispatch: Mark swapped dispatch as _X_COLD
This touches everything that ends up in the Xorg binary; the big missing
part is GLX since that's all generated code. Cuts about 14k from the
binary on amd64.

939ce0bae6 xv: fix double free in AddResource failure case
XvdiDestroyVideoNotifyList already frees the list if AddResource fails,
so don't do it twice.  And set tpn->client to NULL explicitly to avoid
confusing uninitialized memory with a valid value.

912f1fe2bb Xv: Only stop the adaptors when the Pixmap is finally destroyed
Pixmaps are reference counted and DestroyPixmap is called for the
removal of every reference. However, we only want to stop the adaptors
writing into the Pixmap just before the Pixmap is finally destroyed,
similar to how Windows are handled.

63bb5c5ef1 xv: Add missing gcstruct.h include
Commit ea3f3b0786 (xv: Move xf86 XV color key helper to core.) added
code that uses internals of struct _GC. This structure is defined in the
include/gcstruct.h header which wasn't included by the source file, only
gc.h was. That caused the following build failure:

	  CC       xvmain.lo
	Xext/xvmain.c: In function 'XvFillColorKey':
	Xext/xvmain.c:1114:13: error: dereferencing pointer to incomplete type
	         (*gc->ops->PolyFillRect) (pDraw, gc, nbox, rects);
	             ^

Fix this by including the correct header file.

cb42805c66 xv: Move common code for adaptor cleanup to xvmain.c

Xv used to call XvScreenInit and co. through function pointers, as
XvScreenInit may have been sitting on the other side of a module
boundary from xf86XvScreenInit.  Why this was so is a mystery, but make
