Move functions/macros dealing with request parsing or reply assembly/write
out of the big dix_priv.h into their own headers. This new header will also
get more of those function/macros soon (yet still in the pipeline).

In Xinerama/Panoramix configuration there's one screen that's having special
meaning - it's used for simulating as the frontend for all client operations:
the clients (should) only talk to that screen, while panoramix subsystem is
proxying those operations to all the other screens (with certain changed
applied, eg. coordinate transformations).
Historically, this screen happens to be the first one in the system (some of
it's proc's are hooked up in order to achieve desired behaviour). That's why it
used to be accessed via screenInfo.screens[0] - that already had been encapsulated
into a tiny helper `dixGetFirstScreen()`.
a) the correct terminus technicus for a situation where one device (or SW entity)
entirely controlling others is a master-slave-relationship: the controlling
device/entity is `master`, the controlled ones are `slave` (to that specific
master).
b) the term "first screen" is inacurate and misleading here: what the caller's are
actually interest in isn't the first entry in the screen array, but the screen
that's controlling the others. With upcoming refactoring of the Xinerama/Panoramix
subsystem, this might well be a different array index than 0.
c) the term `default` also wouldn't match: `default` implies there's a real practical
choice, and such value applies when no explicit choice has been made. But in this
case, it practically doesn't make sense (except perhaps for debugging purpose)
for a client to use any different screen.
Therefore fixing the function name to the correct technical terminology.
(for sake of patch readability, renaming corresponding variables is left to
subsequent patches).

Instead of everybody directly accessing the (internal) screenInfo struct,
let those consumers only interested in first screen use a little helper.
Also caching the value if it's needed several times.

Since we're already statically initializing the reply header anyways,
use the default case values here and overwrite them only when requested
version is lower than server's one.
That code path is so cold (at max only called once per client), we really
don't need to care whether there's any chance for saving a few cycles.
So not trying to be clever, but instead focus on readability.

The dispatcher functions are much more complex than they're usually are
(just switch/case statement). Bring them in line with the standard scheme
used in the Xserver, so further steps become easier.
It's also much cleaner to use the defines from proto headers instead of
raw numbers.

The current way of switching between Xinerama and single-screen handlers
is quite complicated and needs call vector tables that are changed on
the fly, which in turn makes dispatching more complicated.
Reworking this into a simple and straight code flow, where individual request
procs just look at a flag to decide whether to call the Xinerama or single
screen version.
This isn't just much easier to understand (and debug), but also removes the need
or the call vectors, thus allowing further simplification of the dispatcher.

Using calloc() instead of malloc() as preventive measure, so there
never can be any hidden bugs or leaks due uninitialized memory.
The extra cost of using this compiler intrinsic should be practically
impossible to measure - in many cases a good compiler can even deduce
if certain areas really don't need to be zero'd (because they're written
to right after allocation) and create more efficient machine code.
The code pathes in question are pretty cold anyways, so it's probably
not worth even thinking about potential extra runtime costs.

FOR_NSCREENS() is just alias for FOR_NSCREENS_BACKWARD(). In many cases
it really matters that we're going backwards and the last iteration visited
the screen #0, and that one is panoramix-wrapped.
Thus directly calling FOR_NSCREENS_BACKWARD() here and dropping the alias.

PANORAMIX was the original working title of the extension, before it became
official standard. Just nobody cared about fixing the symbols to the official
naming.
For backwards compatibility with drivers, the old PANORAMIX symbol will
still be set.

The symbol controls whether to include dix-config.h, and it's always set,
thus we don't need it (and dozens of ifdef's) anymore.
This commit only removes them from our own source files, where we can
guarantee that dix-config.h is present - leaving the (potentially exported)
headers untouched.

Since we already had to rename some of them, in order to fix name clashes
on win32, it's now time to rename all the remaining ones.
The old ones are still present as define's to the new ones, just for
backwards compatibility.

Windows' native headers using some our RT_* define's names for other things.
Since the naming isn't very nice anyways, introducing some new ones
(X11_RESTYPE_NONE, X11_RESTYPE_FONT, X11_RESTYPE_CURSOR) and define the old
ones as an alias to them, in case some out-of-tree code still uses them.
With thins change, we don't need to be so extremely careful about include
ordering and have explicit #undef's in order to prevent name clashes on
Win32 targets.

This touches everything that ends up in the Xorg binary; the big missing
part is GLX since that's all generated code. Cuts about 14k from the
binary on amd64.

The previous change removed the special case that matched resources of
CompositeClientWindowType and walked back from that to the window
pixmap. That was intentional, since that logic was broken anyway. CCWTs
don't map 1:1 to references on the backing pixmap; a window redirected
by multiple clients (say, by the server since it's on the synthetic
visual, and then manually by a compositor) would have a window pixmap
refcount of 1, but would have those bytes accounted twice.
The right thing is to have Composite wrap window accounting, and add the
pixmap bytes once and only once for the redirection reference.
Note that the view from the client can still be non-intuitive in the
face of Composite. xcompmgr, for example, holds _two_ references to
each window pixmap (one each from CompositeNameWindowPixmap and
RenderCreatePicture), so a synthetic-visual window will have its bytes
split 2/3 to xcompmgr and 1/3 to the server-client. Nothing to be done
about that, and at least this way we're not over-accounting.

Casting return to (void) was used to tell lint that you intended
to ignore the return value, so it didn't warn you about it.
Casting the third argument to (char *) was used as the most generic
pointer type in the days before compilers supported C89 (void *)
(except for a couple places it's used for byte-sized pointer math).

This is strictly the application of the script 'x-indent-all.sh'
from util/modular. Compared to the patch that Daniel posted in
January, I've added a few indent flags:

-T _XFUNCPROTOEND
-T _X_EXPORT
The typedefs were needed to make the output of sdksyms.sh match the
previous output, otherwise, the code is formatted badly enough that
sdksyms.sh generates incorrect output.
The generated code was compared with the previous version and found to
be essentially identical -- "assert" line numbers and BUILD_TIME were
the only differences found.
The comparison was done with this script:
